Understanding and Completing Patient Care Reports (PCR)

Who this is for:

EMTs, Dispatchers (review only), Supervisors

Prerequisites:

  • Patient linked to an incident

  • EMT must have active assignment

Purpose of PCR

The PCR documents clinical care provided during transport and is required before an incident can be closed.

PCR Sections Typically Include

  • Patient details

  • Chief complaint

  • Vital signs

  • Assessment findings

  • Interventions provided

  • Transport notes

  • Facility arrival details

  • EMT signature/confirmation

Steps (Mobile App or Web)

  1. Open the active incident.

  2. Tap or click Patient Care Report.

  3. Enter all required fields.

  4. Review for accuracy.

  5. Submit the PCR.

Troubleshooting

  • PCR cannot be submitted → missing required fields.

  • Duplicate PCR created → ensure you’re in the correct incident.
