Merging Duplicate Patient Care Records
Who this is for:
Data Managers, Admins
Prerequisites:
At least two records identified as duplicates
Matching demographic fields (name, DOB, phone number)
Why Merge?
Duplicates occur when patients arrive without ID or when names are spelled differently.
Steps (Administrative Process)
Identify duplicate entries in the Patients list.
Open the primary (correct) record.
Review incident associations.
Open the duplicate record(s).
Manually copy any missing information into the primary record.
Update incidents to reference the primary patient care record.
Mark or archive the duplicate record as inactive.
